This online form may take up to 30 minutes to complete.  We know the process may be time consuming, but very necessary to ensure we can keep your child safe and be mindful of any requests you may have.

We will also send you several more forms by email which are required by law from the State of Florida. All of these forms should be brought with you on your child’s first day.
